See if this works Open your image...go to WINDOW/LAYERS and click on new adjutment layer...looks lika a ying/yang circle bottom of menu. Click on pattern and when the menu comes up load new pattern/artists surface. (Click lil arrow to load different patterns). Load the white surface on the right hand side. Go to Filter/render/cloud and clik OK...blur the layer...choose a paintbrush with a soft edge from the brush menus and with black as the foreground colour...start painting. Change the foreground colour to white to paint it back. Regards

When I hit load new pattern/artists surface I could not choose a white surface. So... while I was looking around I found the gradent tool , so rather than pattern I choose gradent (if that makes sense). then choose a paintbrush with a soft edge from the brush menus and with black as the foreground colour...start painting. Change the foreground colour to white to paint it back. As you sugested. Here is my results Before
After

Matt there's also a filter for this in the excellent, but pricey, Nik Color Efex Pro 2 suite for photoshop. With this filter you choose the effect and just paint it on (click the image to see a larger version).
